The forehead is a central facial feature and an apparent marker of the gender of the individual. A male forehead significantly differs from that of a female. Typically, a masculine face has a more projected forehead (due to brow bridge or brow bossing) which is also larger, wider, and higher. The projection of the forehead is composed of a supraorbital rim (a bony ridge) lying just above and underneath the eyebrow. Their forehead is also sloppy because of the shape of the frontal bone lying beneath it. In contrast, a smooth, vertical, and flatter forehead (with little or no brow bossing) is what characterizes a feminine face.
Forehead contouring (or brow bone reduction) is a facial feminization surgical procedure to reconstruct the prominent forehead (eyebrow) bone and reshape the orbital bone to achieve a more feminine look. It can be done by a combination of approaches such as bone shaving, filling, and reconstruction to get the desired final results.
Best brow bone reduction surgery in India is performed at Designer Bodyz by the expert and experienced facial feminization surgeon Dr. Parag Telang. To feminize the forehead, the most performed brow bone reduction process involves shaving down the solid bone in the outer segments of the eyebrow bossing. However, if the eyebrow bossing is over the hollow frontal sinus, then the surgeon first removes the plate of bone lying in its front, reshapes it, and repositions it using small titanium screws and secures it in place as it heals.
There are four types of forehead contouring described. These are shaving; shaving and filling; forehead reconstruction; forehead reconstruction and filling. The type chosen by the surgeon depends upon the protuberance of the brow bone, the size of the frontal sinus, the general size of the forehead, and the thickness of the frontal sinus wall. In this article, the focus is only on type 1 and type 3 processes.

In the first meeting with the surgeon, the patient is free to discuss their expectations and desired outcome. The surgeon carries out medical examinations and some blood investigations to check the patient for fitness for surgery and anesthesia. The medical history of the patient is reviewed and the face is examined along with soft tissues by radiological assessment. Photographs are clicked of the patient’s forehead for preoperative and postoperative evaluations.
Before commencing with the main procedure, the patient is given general anesthesia. Once it takes effect, the surgeon makes an incision along the hairline of the patient to gain access to the forehead. The periosteal flap is drawn back to expose the underlying forehead bone. Next, a transillumination process is performed by the surgeon to determine the exact shape, size, and location of the frontal sinus. After the thickness of the frontal sinus wall is determined, bone shaving is carried out.
Type 1 forehead reconstruction : The surgeon uses a bone contouring burr to file down the frontal and orbital rims, thereby reducing heavy orbital bone and flattening the prominent supra-orbital bone. It is conducted in patients having small or absent frontal sinus and a thick frontal sinus wall.
Type 3 forehead reconstruction : During this procedure, the anterior wall of the frontal sinus (i.e. outer table of the frontal bone) is cut along the marked lines and removed from its place. The removed portion is remodeled or reshaped on a workstation and then the refined bone plate is put back on the forehead. A titanium mesh or micro-screws are used to keep the remodeled bone in its place.
The forehead reconstruction is covered with a pericranial flap (vascular flap from the scalp) which ensures a good supply of blood to the frontal bone and better healing. Many patients opt for Type 3 forehead reconstruction in India at Designer Bodyz, which is one of the best facial feminization clinics in Mumbai.
